Immunization Requirements

The State of California requires the following immunizations BEFORE a student may begin classes in Pre-K, K and 7th grade.
 
Children entering Pre-kindergarten must have: 
4 DTaP
3 Polio
3 Hep B
1 MMR*
1 Hib*
1 Varicella
TB Risk Assessment (form link below)   
* on or after the first birthday
 
Children entering Kindergarten must have:  (plus see NOTE below)
5 DTaP (4 doses meet requirement if at least one was given on or after the 4th birthday)
4 Polio  (3 doses meet requirement if at least one was given on or after 4th birthday)
3 Hep B
2 MMR (both doses given on or after 1st birthday)
2 Varicella (or physician documentation of disease or immunity)
1 TB Risk Assessment and test results (if indicated) (form link below)   
 
(required within 12 months of registration for Kindergarten for ALL STUDENTS ENTERING KINDERGARTEN FOR THE FIRST TIME and ALL STUDENTS TRANSFERRING TO SANTA CLARA COUNTY FROM ANOTHER COUNTY.)
 
NOTE: When your child enters First Grade NEXT school year (2021-2022), they will be required to submit a REPORT OF HEALTH FORM. You may meet the requirement if you take your child for a physical anytime between now and the start of his/her first grade year (the requirement is within 18 months). Simply have it completed and turn it in so that we may attach it to your child’s health record and he/she will be set for first grade. ** You may want to keep this in mind when scheduling your child's annual appointments.
             
Children entering First grade, in addition to the above, must have on file:
REPORT OF HEALTH EXAMINATION (must be dated March 1, 2019 or after) (form link below)   
 
(This is a record of a health examination given by your child’s doctor. If your child has already had a physical within the time frame and all you need is to have the form completed, you may fax the form to your doctors and have them fax it directly back to the school office. Our fax number is: 408-268-5281.  Otherwise, please take the form with you when your child has his/her health exam and return to the office by August 1, 2020.)
 
Children entering Seventh grade must have:
 3 DTaP (1 more Td/Tdap required if the last dose was given before the 2nd birthday)
 4 Polio  (3 doses meet requirement if at least one was given on or after the 2nd birthday)
 3 Hep B
 2 MMR (1st dose on or after 1st birthday)
 2 Varicella (or physician documentation of disease or immunity)
 Tdap Booster
 
Transfer Students:
If your child is transferring into Holy Spirit School in any grade, K-8, from outside of Santa Clara County, he/she must have the TB Risk Assessment as described above under the Kindergarten requirements (or a TB test.) (form link below)
